Part D covers prescription drugs. Each plan has a formulary — a list of covered medications organized by tiers that determine your cost. Starting in 2025, there's a $2,000 annual out-of-pocket cap on drugs. Never skip Part D just because you're healthy now — the late enrollment penalty is permanent and calculates at 1% per month you go without qualifying coverage.

What Is IRMAA?

Income-Related Monthly Adjustment Amount. If your income reported on your tax return from 2 years ago exceeds $106,000 (individual) or $212,000 (joint), you pay a higher Part B and Part D premium. IRMAA is calculated automatically by Social Security. If your income has dropped recently, you can request a redetermination. A local agent can help you understand whether it applies to you.

Have Both Medicaid (Medi-Cal in California) & Medicare?

If you qualify for both Medicare and Medi-Cal, you're what's called "dual eligible" — and there are special plans built just for you called Dual Special Needs Plans (D-SNPs). Many offer $0 premiums, $0 copays, extra dental and vision benefits, and dedicated care coordinators. You also have more flexibility to change plans throughout the year. Most people who qualify don't know these plans exist.

What Is Medigap?

Also called Medicare Supplement insurance. Original Medicare covers about 80% of your approved medical costs — the remaining 20% is your responsibility, with no cap on what you could owe. Medigap fills that gap. The most popular plans are Plan G and Plan N. Critical timing note: you have a 6-month guaranteed acceptance window when you first enroll in Part B. After that, insurers can charge more or deny you based on health history.

Your Medicare rules depend on your employer's size. If your employer has 20 or more employees, your group plan is primary and you may delay Medicare Part B without penalty. Fewer than 20 employees? Medicare becomes primary at 65 and delaying Part B could trigger a permanent penalty. COBRA does not count as qualifying coverage for Medicare delay purposes — ever. Get this wrong and the penalty never goes away.

If you're already receiving Social Security benefits, you're typically enrolled automatically at 65. If not, you need to sign up during your Initial Enrollment Period. Medi-Cal provides no-premium or low-cost health coverage for California residents with limited income. You can apply anytime — there's no restricted enrollment period. If you have both Medi-Cal and Medicare, you may qualify for a Dual Special Needs Plan that coordinates both programs and could significantly reduce your costs. Federally Qualified Health Centers (FQHCs)

FQHCs are federally funded community health clinics that must provide care to everyone — regardless of insurance, income, or immigration status. They accept Medicare and Medi-Cal, offer sliding-scale fees for uninsured patients, and cannot turn anyone away by law. They often provide primary care, dental, mental health, and pharmacy services under one roof. Programs of All-Inclusive Care for the Elderly (PACE) are for adults 55 and older who need nursing home-level care but prefer to stay living in their community.
